Requesting Information

The process to request information under the Family Violence Information Sharing Scheme (FVISS) and the Child Information Sharing Scheme (CISS), applies to all programs of Mansfield District Hospital who have been prescribed as Information Sharing Entities (ISEs) by the State Government.

Only ISEs are eligible to request or share information under these schemes. To request information from Mansfield District Hospital under either scheme, complete the ‘FVISS & CISS Information Sharing Request Form’ and email it to foi@mdh.org.au

Please note: The onus is on the requesting ISE to seek and gain consent where required under Legislation. Specific details must be provided as to why information is being sought under either FVISS or CISS.

Mansfield District Hospital delegates will review and provide a response within 2 business days.
